What does a Wellbeing Friend do?

BECOMING A WELLBEING FRIEND

Thank you for your interest in being a Community Wellbeing Friend.

We need to collect some basic information from you to enable us to monitor the success of the programme. We only use your data for the means intended. Please see our GDPR statement below for more details*.

Please complete the following information about yourself so that we can build a profile of our Wellbeing Friend who are supporting the communities of Blaenau Gwent, Caerphilly, Torfaen, Monmouthshire and Newport. This will enable us to understand who is signed up, so we can widen access to the scheme as and when needed, ensuring that we have a group of Wellbeing Friends supporting people in all parts of our communities.

The Wellbeing Friends programme is supported by Aneurin Bevan University Health Board Public Health Team. Information submitted may be stored and processed electronically for the purposes of delivering services, supplying information and to enable statistical analysis.

 

Data may be shared to other parts of the organisation, partner organisations (GAVO), companies or contractors operating on our behalf to enable this to take place. We follow appropriate security procedures in the storage and disclosure of personal information so as to prevent unauthorised access by third parties. We also require those parties to whom we transfer personal information to comply with the same.

Please note that as part of signing up to be a Wellbeing Friend and further to the legitimate interests of Aneurin Bevan University Health Board, you will automatically be subscribed to receive updates by email. You can unsubscribe and be removed from the Wellbeing Friends register at any time. You can do this by sending an email to the wellbeing_training.ABB@wales.nhs.uk mailbox stating ‘OPT OUT’ in the subject.
